Description
The input locator to the Add Polygon Fields To Locator tool already has fields from a polygon layer attached to it.
Solution
Remove the polygon layer fields attached to the locator using the Delete Polygon Fields From Locator tool before adding the locator to the Add Polygon Fields To Locator tool.
